Final medical exams in 10 days.. Arrrrrrggggggggggghhhhhhhh! This is just stress relieving procrastination, sharing my stress with the unseen many. Also, was a crazy night last night:

- Meistered several new spinning contact staff trix & transfers (be4 burning hand.. Doh; I be branded on my palm, very pretty!)

- Practiced fire eating and nailed my fire in mouth to torch transfer (on my balcony) (colemans is the key). Was very chuffed with myself, went in to deparrifinate (shower biggrin) when I recieved a phone call from one of my mates who pointed out that she had just driven past and there were no less than TWO fire engines (>10 firemen) outside my block of flats. Swearing (and assuming (quite correctly) it was me they were after) I threw on some clothes and jogged down to explain the situation to the, quite frankly, amused firemen.. They pointed out the nosey neighbour concerned, peeking from behind his curtain (the same guy who called the police having thought I'd hung myself in my kitchen; I'd hung a hoodie up on kitchen door ubblol!)... Gotta laugh!

- Then chopped wedge out of thumb while cooking (doh; the gods of revision want me to stop pissing around on my guitar and do some work)

- Went exceedingly hyper (without aid of coffee for once) with jools and Mr Markus, we set up an amp out of my window with my mic running through fx pedel and then shouted from my darkened louge(with wierd alien voice effect) at the drunks staggering home ("goodevening pleasure seekers, if you look to your right you will see..... a car".. clanger impressions.. Shouting @ the "peasants" to "fear our wrath" etc etc etc.,)... Hmmmm; and may I just say for the record...... MALT!!!!!

Anyhoo.. I must go and finalise last few things for my elective in J'berg (only 20 days to go!!), pick up my neutral density filter (expect cool fire pics), and pretend to revise in several of the coffee bars in town!

10 days.... and counting....

Sitting here making micropoi to switch brain off (just learnt monkeyfist knot), hopefully i can play with them tomorrow and avoid all the other stressed medics doing last min cramming and dissecting the exam (always a headfuck)

Well, 5 years working to this point.. Not much more to do. There will be muchos partyiage come thurs; find out if i qualify mon, then j'berg weds.

What's your exam in? Or is this a final qualifying exam?

I have Step II of the Boards this summer. 9 hours of sheer multiple choice joy. They've also "blessed" us by making us the first class to have to do a clinical exam, where we have to interview standardized patients.

A patient presents to the emergency department with crushing substernal chest pain. Which of the following would be MOST LIKELY to yield a diagnosis?

a) electrocardiogram
b) serum troponin and creatinine kinase (myocardial fraction)
c) chest X-ray
d) physical examination
e) history

That's pretty much every question. Not enough information and basically you have to read their minds. The answer, of course, is that I'd order all of these STAT and I'll take whichever comes first.

Isn't it fun? Just remember, we're doing this because want to help people!

but thats just me, i seriously don't know how you guys put up with the amount of shyte they put on you in college and uni...i couldn't put up with it anymore.

When I was younger, "all of the above" was the guaranteed right answer if it was offered.

Not so anymore. It's often offered as an answer to all questions. And if it isn't, then it's the wrong answer.

This is for the BMBS which lets me become a PRHO and replace my Mr for Dr (and an income; i have my eye on Somoan fire knives). 5hours over two 2.5h MCQ exams covering all Medicine, Surgery, Orthopaedics and rhumatology. I have clinical exams on weds/thurs. Up early to get going; the plan is:

- Good coffee, good breakfast
- Rock out on electric guitar mad
- Stick on Matrix II soundtrack and spin like a demented Monkey with Menieres disease ubbrollsmile
- Walk in
- Awoid other medics

P.S In the situation described the MOST likely to yield a diagnosis is ECG (was that C); And is the one that is really gonna dictate your management.. In real life it is all of the above; Right, time for step A of the masterplan

Yes, just had long case with patient who had psoaritic arthritis with autoimmune liver disease and oodles of signs... Trouble is i rhum puts me to sleep.. I like acute medicine; Hearts, Neuro, Ortho etc.,... After the trouble on Mon with a good 25% of the papers being devoted to WIERD ASS rhumatology conditions (It just happens to be set by rhumatologist; you couldn't tell wink) I had already moaned about my dislike (or rather dislike fuelled by lack of knowledge) on this condition; I kinda know it.. But I refer you to original point..

My Prof Voodoo Doll (for learning aches and pains) is going to get some severe usage.. For this he deserves... EVERYTHING.. I'll nail it to the bloody wall and throw knives at it.. Before ritualistic burning ubbloco

I have ankylosing spondylitis. The reason it's important to know about this stuff is because I got bounced around from doctor to doctor for six years with "low back pain of uncertain etiology." I wound up on Norpramin (desipramine) prescribed by a psychiatrist.

Now, when I was 12, a CTLS X-ray showed inflammation of the SI joints. From when I was 13 to 15, I woke up every morning with this gnawing low back pain that got better with time. Now, you'd think it would have occurred to someone that this constellation might warrant a sed rate and a HLA typing. Well, my sed rate was 80 and I am HLA-B27. And my father had AS, as well.

And I never would have been called a crazy hypochondriac.

I'm all for recognizing it; I'm big fan of clinical skills (the lost art of examination, the more obscure or strangley named the better) and diagnosis. But it is a specialisit management; and clearly not everyones cup of tea. I realise it's important to know and importantly to RECOGNISE; but when they get to the nitty gritty of management etc it can get quite complex with NSAIDs, DMARDs, multidisciplinary approach (which is where I struggle). Sorry if came accross as bit flippent; Exam come down, I have a hold of myself now.

Example of exam questions I'm bitching about:

Or my favourite was on the X-Ray changes associated with psoaritic arthritis. Now I would recognise sclerosis, bone lesions, erosions, periosteal reactions etc etc., and would prob refer (or if I saw psoarisis put 2 + 2 together), BUT (and this is where I have a problem); To devote 10 stems to the X-ray changes of psoar arth and have it where your trying to differentiate the SUBTYPE of erosion is ludicrous! This is Undergrad level, those are postgrad, rhum questions... Most SHOs i know wouldn't know!

I get annoyed at people who get passed from pip to post. I have a few friends like that whose GPs refuse to believe there is something wrong and I KNOW there is..
